form is a polyphonic sampler. It uses a lemur layout as a control source which is connected via OSC (no setup), you will need lemur though

for setup: make an OSC target on your ipad and use the matching port number on the max patch. little white led should shine if info is coming through

--after setting up connection load an empty preset and then a couple of taken ones so the max patch syncs with the lemur
